Elevated Health & Wellness is a functional primary care practice founded and led by Keyona Gayles, NP-C, a board-certified nurse practitioner based in Tinley Park, IL and virtually. Our mission is to bridge the gap between conventional medicine and whole-body wellness by addressing the root causes of health concerns rather than simply managing symptoms.

We specialize in functional medicine, GLP-1 weight loss medication management, hormone optimization, IV hydration therapy, women’s wellness, preventive care, lab testing, and peptides. All care is delivered with a personalized, patient-first approach. We also offer telehealth services, vitamin injections, nutritional counseling, and access to professional-grade supplements through our Fullscript dispensary.

Our ideal patients are adults who is ready to take an active role in their health, feel their best, and receive care that actually listens. We accept most PPO insurances and offer a Direct Primary Care membership for those who prefer a more flexible, concierge-style experience.
